Caprock Home Health Services is the largest family-owned home health provider in Texas. CHHS was established in 1983 and has been helping our patients live independently from the comfort of their own home for nearly 40 years.

Nursing

Home health nursing is a specialty in which nurses provide multidimensional home care to patients of all ages. Home health care is a cost efficient way to deliver quality care in the convenience of the client's home. Home health nurses create care plans to achieve goals based on the client's diagnosis.

Therapy

Physical home health therapy sessions help patients understand how increased physical activity and better lifestyle choices can lead to weight loss, lower blood pressure and improved mental health. An exercise program may also decrease future cardiac events and improve mortality. Occupational and Speech Therapy can also be managed at home.

Personal Assistant Services

PAS is a benefit that helps clients with everyday tasks. These tasks are called activities of daily living and instrumental activities of daily living.

Remote Patient Monitoring (RPM)

RPM is a technology to enable monitoring of patients outside of conventional clinical settings, such as in the home or in a remote area, which may increase access to care and decrease healthcare delivery costs.
